Medication Therapy Management and Medication Reconciliation

What is it?

Medication Therapy Management and Comprehensive Medication Review and Reconciliation are services provided by the pharmacist to help you better manage your medications. During this service, a pharmacist will review your health and medication profile, including OTC medications, to assess for appropriateness and adherence.

Do I need one?

All patients, especially those with multiple medication or chronic conditions, can benefit from a medication reconciliation and review. 

Some insurance cover MTMs at no cost to the patient.
